“My signature indicates that I have read and agree to the terms and conditions of my chosen payment plan and the payment plan agreement. I understand that my chosen option will remain in effect during my time with Sting Soccer Club. I will contact Sting if any changes are necessary. I agree to remain in good standing by paying my monthly dues on time. I understand that if I do not make the agreed upon payments by the due dates my scholarship could be in jeopardy and my account is subject to collections. All money that I have paid will be forfeited if I default on this agreement.
For Sting Soccer players:
- For every $100 awarded in financial aid, one hour of Sting Soccer–approved volunteer service is required.
- I acknowledge that the scholarship will only reflect on my invoice once the balance I owe is settled in full. I am aware that the awarded amount will post as my last payment, and I’m responsible for all outstanding payments before the scholarship is applied towards my dues.”
- In the event that a player withdraws from the club before the end of the season, any financial assistance awarded shall become immediately due and payable in full by the player’s parent or legal guardian.
